Please also fill up the w8 (for International Call Centers / Publishers) or w9 (for US Call Centers / Publishers), available ONLINE at http://blog.zenfinancial.com/wp-content/uploads/2015/09/fw8ben.pdf (w8) or http://blog.zenfinancial.com/wp-content/uploads/2015/09/fw9.pdf (w9) which is relevant to you, and send it back, along with the signed Agreement.
Please make sure, that you send back the signed Agreement and w8 / w9, ASAP, failing which we shall be deactivating your account.
